the following are requirements for Foreigners who are previously a Filipino Citizen
1. 2 Photocopy of any Gov't issued ID's with picture not expired ( Husband & Wife ) with 3 specimen signature
Acceptable ID: Driver's License ID, SSS ID, GSIS, Unified ID, Passport ID, Postal ID, TIN ID's
2. 2 Different Original Proof of Billing Address
Ex. Water Bill, Meralco Bill, Cellphone Bill, Credit Cards
3. Copy of Marriage Certificate
4. TIN ID or fillep-up and signed BIR Form 1904 (click here to download the form) with attached copy of buyer's Birth Certificate.
5. recently filled up and notarized sworn declaration (click here to download the form)
