1. For BPI Application

BASIC REQUIREMENTS:
· Signed application form and authorization letters
· 2 valid government issued IDs
· Proof of billing – Philippine address

FOR LOCALLY EMPLOYED
· Certificate of Employment indicating salary, position, and length of service
· Latest ITR
· Latest 3 months payslips
· If possible: 3 months bank statements reflecting salary credits
· If practicing doctor affiliated with hospitals: clinic schedule and to submit latest 6 months bank statements
FOR SEAFARER
· Certificate of Employment indicating salary, position, and length of service if possible
· Latest POEA Contract
· Crewing Officer name and contact details

FOR SELF-EMPLOYED
IF EARNING FROM RENTAL INCOME
· Photocopy of Title
· Lease Contract (indicating full address of property rented out, name of tenant and rental amount)
· Latest ITR
· Bank statements (latest 6 months proof of rental payments)
IF EARNING FROM COMMISSION
· Bank statements (latest 6 months proof of commission)
· Latest ITR
· Certificate of employment from affiliated companies
IF EARNING FROM TRANSPORT BUSINESS
· OR/CR, LTFRB Franchise and Mayor's Permit
· Latest ITR

FOR BUSINESS ENTITIES
· Single Proprietorship: DTI Registration and/or Mayor's Permit
· Corporation or Partnership: SEC Registration, Articles and By-Laws
· Latest ITR with Audited Financial Statements for 2 years
· Latest 6 months Bank Statements – working bank account (6 mons to 1 yr latest bank statement)
· List of Major Trade References: atleast 3 clients or suppliers to provide company name, company address, contact person and contact number

FOR WORKING ABROAD
· Certificate of Employment indicating salary, position and length of service
· Proof of yearly visits to the Philippines (passport stamps)
· Latest 3 months payslips
· If possible: 3 months latest bank statements reflecting salary credits
· If possible: 3 months latest proof of remittance
· Contact person in Philippines: (Loan administrator/SPA): Name, address, contact number and relationship to the borrower
· Valid government issued ID of Contact person
· If based in USA: to provide US Federal ITR
· To submit upon approval:
> Consularized/Apostilled/Notarized BPI Family Savings Bank’s Special Power of Attorney (SPA) format
> Consularized COE and latest 3 months payslips

Please note additional requirements:
· SINGLE - Certificate of No Marriage NSO/PSA certified with official receipt (to submit upon approval)
· LEGALLY SEPARATED - court decision/Finality of Marriage and Marriage contract with annotation of annulment with Official Receipt
· MARRIED - Marriage Certificate NSO/PSA Certified
· FOREIGN CLIENTS  - will only allow if married to Filipino citizen holder and only income of Filipino borrower will only be used
· MINOR CLIENTS - Court approval of Guardianship

Property to be mortgaged documents (if available)
· Accredited Developers - Signed Contract To Sell
· Individual Seller ­– Owner’s Duplicate Copy of Title and tax declaration

2. For China Bank Application

If you have applied for a Chinabank Housing Loan, you will be asked to provide some supporting documents in order for the bank to process your application. These documents are listed below:
If Locally Employed:

• Application Form with attached 2x2 ID picture
• At least 2 Valid ID's
• Marriage contract (if applicable)
• Latest Income Tax Return
• Certificate of Employment with Compensation and Tenure

If Self-employed (sole proprietorship):

• Application Form with attached 2x2 ID picture
• At least 2 Valid ID's
• Marriage contract (if applicable)
• DTI registration
• Audited Financial Statements for the last 3 years
• Bank Statements
• Company Profile
• Complete list of customers and suppliers with contact person and number

If Self-employed (partnership/corporation):

• Application Form with attached 2x2 ID picture
• At least 2 Valid ID's
• Marriage contract (if applicable)
• SEC Registration, Articles of Incorporation, and By-Laws
• Audited Financial Statements for the last 3 years
• Board/Partnership Resolution / Secretary's Certificate
• Current General Information Sheet (GIS)
• Company Profile
• Complete list of customers and suppliers with contact person and number
